Insurance Information

PLEASE NOTE:  Effective September 1, 2008 We will no longer be a participating provider with Blue Cross/Blue Shield. We  will continue to provide services to your child /children as an out of network provider.Payment is expected at the time of service and we will give you the  information required to get reimbursment from your insurance company. Please call our office should you have any questions.We continue to be a contracted provider for-Aetna,Cigna,Humana,PHCS,Elmcare HMO and Beech Street insurance.Please call us if  you have any questions about your insurance.

Please have your referring physician fill out the consultation form before your visit so that we may send a consultation report to them.
